How to encourage physical activity among people in Nice being treated for a Long-Term Illness (LTI)* by offering them a complimentary physical and motivational assessment, directing them to a partner structure, and providing follow-up?

NICE ACTI’SANTE, a coordinated scheme implemented with all relevant medical and social stakeholders, is integrated into a medical-active pathway with the entry point being the general practitioner or specialist.


How to participate?

Patients are taken care of on the NICE ACTI’SANTE platform by a qualified teacher in adapted physical activity and receive a complimentary assessment of their physical condition (balance, flexibility, strength, and endurance tests) as well as an interview to identify their expectations in terms of physical activity and potential barriers to practice. This assessment will allow the teacher to guide the patient towards a partner structure offering a physical activity program tailored to their health status and lifestyle, and taking into account their personal aspirations, capabilities, and needs.
An evaluation of early signs of frailty will also be offered to patients over 65 on the frailty platform of Nice University Hospital, located at the Cimiez site.

What is the procedure?

After reviewing the assessment established by the teacher of the NICE ACTI’SANTE platform, the doctor prescribes physical activity to the patient and issues a certificate of no contraindication to the recommended practice.

The patient is then taken care of by the recommended partner structure. Concurrently, the NICE ACTI’SANTE platform team sets up personalized follow-up for the patient to assist them in adopting regular and sustainable physical activity (phone calls, SMS, emails, invitations to individual or group therapeutic education sessions).

At the end of their care in the partner structure, the patients are invited by the teacher from the NICE ACTI’SANTE platform to undergo a new assessment of their physical condition and an interview to guide them this time towards a health and wellness sports offer or in a regular environment for independent practice.

What are the access structures?

The City of Nice and its partners* aim to make the NICE ACTI’SANTE platform a genuine local service for the most vulnerable inhabitants of Nice. For this reason, it will be set up at 4 different sites to be as close as possible to the residents:

  • NORTH Site > Every Monday afternoon at AnimaNice Gorbella Center (65 Bd Gorbella)
  • CENTER Site > Every Tuesday morning at Jean Bouin Sports Complex (2 Rue Jean Allègre)

  • WEST Site > Every Tuesday afternoon at Mercantour Sports Complex (114 Bd du Mercantour)

  • EAST Site > Every Wednesday afternoon at 27Delvalle (27 Rue du Professeur Delvalle)

For the record

LTIs are an increasing challenge in human, health, and financial terms: nearly 8 million people in France today are affected by one of these 30 conditions, including diabetes, heart failure, HIV, Parkinson’s disease, and Alzheimer’s disease. The number of patients has been increasing by more than 5% per year for the past 10 years. The practice of adapted physical activity could help limit this growth and allow those affected to maintain their health and slow the progression of the disease.
